Vacation Request Form for Exchange 2010

Are there any Vacation Request Forms available using Exchange 2010 sp3 and Outlook 2010/2013? 

1. open up the form and fills in the details, send to the authorized director
2. the director will then open up the form and decides whether to APPROVE or DENY
3. if DENY, send notification to sender
4. if APPROVE, post appointment to a public folder called "Calendar - DIS" and send notification to sender that it was APPROVED.
